MARC Report 255: Program changes

A change was made to improve the reliability of the Upgrade MARC-8 to UTF8 option (sometimes it worked and sometimes it did not, depending on how the marcxml qualifier was initialized).

Another change was made to disable the option referred to above for records with leader 09 set to 'a'.
